The centre matters because the list is published per Saudi mission: Islamabad, Lahore, Peshawar and Multan follow the Islamabad Mission, while Karachi and Quetta follow the Karachi Mission, and the two lists are not the same.

Tasheer publishes the checklist for a Saudi mission, so a Karachi or Quetta applicant can be asked for a different set of documents than an Islamabad, Lahore, Peshawar or Multan applicant for the very same visa type. Originals, copies and attestation
Most categories ask for the original passport, a clear copy and CNIC copies, and several ask for relationship or sponsor documents attested by the Pakistan Ministry of Foreign Affairs. Read the exact wording on the selected list before paying for any attestation.
Medical and vaccination items
Resident applications can also require a GAMCA medical report and a vaccination card, and the age thresholds differ between the Islamabad and the Karachi mission. Treat the specific line on the selected checklist as the requirement rather than any summary of it.
